What insurance plans do we take?

What insurances plans do we NOT take?
We do NOT accept Blue Cross and Blue Shield: Florida Blue/Anthem Insurance — Amerihealth: Caritas — Humana: HMO/HK — Simply: HMO/HK – Preferred: HMO/MMA— Sunshine: Ambetter.

Do we offer self pay packages?
Yes , we have a self pay package offered.
Therapy
Initial Visit: $175.00
Follow up Visits: $150.00
Marriage/Family Therapy: $200.00

Where do clients park?
Our office building perimeter is street, pay by phone parking. These are the best options as the open lots are reserved for building tenants. Please note your car may be stickered if a client parks in a reserved spot.

Do we have any additional fees?
Yes. We charge a No Show / Late Cancellation fee for individuals with a Non-Medicaid insurance plan. This is to help compensate our staff as their scheduled time is valuable.

We also charge a $10 fee for medical records to be printed and provided upon request.

Does our psychiatrist provide services to children?
Our psychiatrist is currently seeing patients 16 years old and older.

Do we do ADHD evaluations?
Our licensed assessors may complete a biopsychosocial evaluation to determine needs for services, however we do not complete evaluations for school accommodations. Our assessments are for recommendations related to mental health services to be provided.

Do we diagnose ADHD and Autism?
Our licensed assessors may complete a biopsychosocial evaluation and determine a diagnosis of ADHD if applicable, however are not able to diagnose autism. Our recommendation is to seek an evaluation from a neurologist for this level of assessment.

Who is financially responsible for the services provided?
As an agency, we bill the insurance information provided for the services conducted. If the insurance does not pay for the services due to any reason they see fit, the client is responsible for the financial responsibility and an invoice will be sent accordingly.

Do we provide ABA services for adults?
Our ABA services are limited to individuals between the age range of 2 and 21 years old.

What if I do not want to sign off on required documentation?
If an individual does not want to sign off on required documentation (e.g. consent forms, treatment plans, service plans) we reserve the right to close the case for services as this implies the individual does not consent to the service.
